Thank you for choosing to come back for another summer at Crossroads for Kids. Crossroads is built on the relationships that our staff have with the kids and teens in our programs. These relationships can only benefit and flourish when our seasonal staff make the decision to return for another summer! We hope you get just as much out of the relationship and that you are ready for another inspirational summer at Crossroads for Kids!
Please note that the information on this page is focused towards our returning staff members who worked with us during the summer of 2012 and have received a 2013 Staff Agreement.
Paperwork Checklist for Returning Staff
To begin to prepare for your return to camp, please complete and return the following forms together with your signed staff agreement to Annmarie Quinn by email (email@example.com) or mail (HR Department, Crossroads for Kids, 119 Myrtle Street, Duxbury MA 02332, USA):
Health Form (note that an ACA regulation change means that we now require that every staff member has a physical exam from their Primary Care Physician every year)
We advise that you also bring a copy to staff training.
What to bring to camp
Bring originals or legible copies of the following to staff training (or submit with paperwork above):
• Passport or two forms of ID (one must be a photo ID)
• Social Security card
• Health Insurance card
• Physical (completed after July 1st, 2012)
• DS2019 with passport (international staff only)
