Medical Information
Individual medical formsIt is the group leader’s responsibility to gather important guest information, including: name, age, address, emergency contact names and phone numbers, and a list of any known allergy or health condition requiring treatment. If they are minors, signed permission to seek emergency treatment from the parents or legal guardians, prior to staying at camp is also required.
Dispensing MedicationsParticipant medications are the responsibility of group leaders. You are asked to designate a nurse, parent, or other adult as the medical contact and who is responsible for all group member’s medications. Crossroads Connects staff are not responsible, or able to give medication to participants.
Designated Medical StaffWhile here, you are responsible for first aid. We advise group leaders to have adults with First Aid and CPR certifications from nationally recognized providers on duty for emergency care while at camp. Please bring your own first aid supplies and equipment. Camp will provide somewhere to store the supplies if needed. A limited amount is available at camp.
InsuranceGroups and their participants must provide their own liability and accident insurance coverage. Groups using camp must provide proof of insurance. Group leaders are given a required Emergency Orientation upon arrival with information on the closest First Responders and how to call in the event of an emergency. In the event of an emergency the group is responsible for driving a participant if he or she needs transportation to/from the doctor/hospital. Local emergency medical services are aware of Crossroads for Kids’ location and will quickly respond to a 911 call when required. In the event that an ambulance is called, the group is responsible for providing someone to ride with the participant to the hospital.
Incidents and AccidentsThe Director of Crossroads Connects must be notified of any accidents requiring medical attention, incidents requiring follow-up, and accidents regarding camp safety that were not attended to by a Crossroads Connects staff member. Please write down the specifics of the accident/incident and then meet with our staff to fill out a detailed report.

In the event of an emergency
If the emergency is of a life-threatening nature call 911, and alert a Crossroads Connects staff member immediately. Otherwise, contact a Crossroads Connects Director.

South Shore Hospital is easily accessible from Route 3 North toward Boston Take exit 16 off Route 3 on to Route 18 south (Main Street). At the third traffic light, turn left on to Columbian Street. Turn right on to Fogg Road, pass the hospital's emergency entrance on your right, and then turn right into the main entrance parking lot.

Jordan Hospital is easily accessible from Route 3 South toward Cape Cod Take Exit 5 (Long Pond Rd.) off Route 3, and, at the bottom of the ramp, turn left and go under Route 3. Take the first right onto Obery Street. After passing the Registry of Deeds on the right, the Jordan Hospital access road is the next right, directly across from the entrance to Plymouth North High School. Free valet service and parking are in front of the main entrance. Tipping for the valet is optional.
